For those who can’t fall asleep…
People with high stress lives run the risk of over active adrenals or adrenal hyper-function. Since this describes the lives of most Americans, most readers at some point during their life, have periods where they can’t fall asleep. When one has ANY kind of stress whether it is mental, emotional, chemical, toxic, food sensitivities, or organ disease and malfunction the body will need extra cortisol to pay for the increased metabolic needs caused by the stress; often, many of these factors exist simultaneously. Typically, they have higher than optimal cortisol levels when bedtime arrives, due to the excitatory relationship between cortisol and the central nervous system. High cortisol levels at bedtime are a prescription for insomnia. The reason for high cortisol levels is simply because the body is being put in a stress response state which fires up the adrenals to compensate. This over compensation means that you don’t sleep. Pretty simple… Or so it seems. Since cortisol stabilizes blood sugar during the day, it is normally released between meals to balance blood sugar. When blood sugar or glucose levels drop, the Hypothalamus/Pituitary/Adrenal Axis (HPA) causes the release of cortisol to maintain glucose levels. Normally, cortisol levels should be high in the morning and low at night, dropping gradually throughout the day. In abnormal circadian cycles, the cortisol levels usually stay high all day to balance blood sugar levels.

If high stress and high cortisol levels exist long enough, the individual may progress from Hypoglycemia (low blood sugar) to Insulin Resistance and later to Diabetes. During this progression, they may suffer from the symptoms of high blood sugar, where the body becomes flooded with insulin. In this phase, people begin to crash after a meal feeling sleepy and fatigued, especially when eating a high carbohydrate meal. They also have intense cravings for sweets after meals.

In a stressed out America, many people have trouble falling or staying asleep. The standard medical model of treatment involves pharmaceutical intervention. Quite frankly, not only are these medications poorly tolerated by the individual; but also, drugs are a short term masking of the problem which must eventually be addressed at its origin.

The shocking truth is that law enforcement agencies are confirming what research has also indicated – sleep deprivation is responsible for more vehicular injuries and deaths than drunk driving.  What’s going on?

Sometimes insomnia is directly related to Hypoglycemia (low blood sugar), Insulin Resistance (high blood sugar), or full blown Diabetes. Almost always when blood sugar imbalances exist, there is stress on the adrenal glands. The adrenal glands are two pea sized glands located above kidneys which produce hormones that will stabilize blood sugar. When the adrenals secrete cortisol in higher than normal amounts at bedtime, the pattern is called adrenal hyper-function (overactive adrenals) resulting in the inability to fall asleep. Adrenal hypo-function is just the opposite in that patients can fall asleep but not stay asleep. Obviously, both conditions will impact sleep.

Testing for adrenal function involves a test called the Adrenal Salivary Index (ASI). The individual will take 4 saliva samples during the day and send it to the lab for analysis. The lab can see the circadian cycles of cortisol throughout the day to determine what stage of adrenal stress the person is in. The test is accurate and reasonably priced.

There are crucial relationships between the thyroid gland and the reproductive/endocrine systems in males and females.

In females, thyroid imbalances  lead to  pituitary-gonadal dysfunctions which include alterations in FSH and LH hormone coordination and release, ovarian FSH receptor site sensitivity, and peripheral progesterone receptor sensitivity. These imbalances lead to miscarriages, ovarian cysts, infertility,  migraines, weight gain, mood disorders, depression, and abnormal menstrual cycles. Sometimes the endocrine symptoms are so pronounced that the thyroid culprit is overlooked. Conversely, the use of synthetic hormones such as birth control pills or hormone replacement therapy (HRT), in addition to insulin surges and stress responses, lead to abnormal spikes of hormones such as estrogen, cortisol, and testosterone, which disrupts health thyroid function.

In males, thyroid imbalances can lead to pronounced  male hormone imbalances which include decreased testosterone, altered pituitary-gonadalfunction, obesity, exhaustion, cognitive decline, erectile dysfunction, and depression.  This clinical presentation will largely be ignored because the male hormone imbalances will obscure the thyroid culprit. Many men start taking synthetic testosterone which further disrupts thyroid metabolism.

At Brown Chiropractic  Neurology, we look for the root cause of ANY metabolic disorder. As you can see, sometimes the problems are not where the symptoms present.

Take this test to help determine what type of adrenal function you have.  Answer  0 (never), 1 (infrequently or sometimes), 2 (frequently), 3 ( most or all of the time).

HYPO-ADRENALISM

Slow starter in the morning                             0, 1, 2, 3

Afternoon fatigue                                                0, 1, 2, 3

Can’t stay asleep                                                   0, 1, 2, 3

Crave sweets                                                         0, 1, 2, 3

Afternoon headaches                                       0, 1, 2, 3

Exertion headaches                                          0, 1, 2, 3

Dizziness when quick to stand up                0, 1, 2, 3

Weak nails                                                              0, 1, 2, 3

If you answered mostly 2’s and 3’s, it is probable that your Hypothalamus-Pituitary-Adrenal (HPA) axis not producing enough cortisol and your adrenals are under-active.

HYPER-ADRENALISM

Wake up tired after 6 or more hours sleep               0, 1, 2, 3

Can’t fall asleep                                                                     0, 1, 2, 3

Perspire easily                                                                      0, 1, 2, 3

Excessive perspiration with no exertion                  0, 1, 2, 3

Weight gain under stress                                                 0, 1, 2, 3

Under high amounts of stress                                       0, 1, 2, 3

If you answered mostly 2’s and 3’s, it is probable that your HPA axis is producing too much cortisol and your adrenals are over-active.

The best method of determining the right diet, lifestyle, and nutritional supplementation for your individual case is to do an ADRENAL SALIVARY INDEX (ASI) test. This would be done after a comprehensive blood panel ruled out physical disease states (such as Anemia) which could be exhausting the adrenal glands.

So many patients report to our office with complaints such as headache, neck pain, back pain, and extremity pains. Others report with neurological conditions such as vertigo, balance disorders, traumatic brain injury, tourette’s, tremors, or ADD/ADHD. But when they list their secondary complaints, frequently the words fatigue or chronic fatigue (CFS) pop up. Interestingly, fatigue is down the list rather than their fist complaint. I strongly feel that the reason is that fatigue goes along with so many seemingly unrelated complaints is that pain or dysfunction can stress the person’s metabolic system. An example: someone is under high stress and begins to suffer from adrenal exhaustion. This results in disruption on cortisol levels leading to insomnia. Without proper sleep, the person will suffer breakdown on mental and physical vigor. The list of possible conditions causing fatigue goes on and on. Anemia, thyroid dysfunction, autoimmune disease, sluggish liver and gall bladder, blood sugar imbalance, and so forth. Many times there is a breakdown of more than one of these areas at once – such as adrenal fatigue leading to insulin/blood sugar problems such as hypoglycemia or insulin resistance.

There are many quick fix elixirs out there, especially in multi level marketing, that help people with minor problems but many times there needs to be a more individualized approach. At Brown Chiropractic Neurology, our approach is the obtain a good history, perform a comprehensive neurological and metabolic exam, and do a complete blood work to determine where the fatigue comes from. If you suffer from chronic fatigue or fibromyalgia, I can help.
